Rental Policies/Terms and Conditions

Cleaning Information: We want you to know that we take cleaning and disinfecting seriously, and our cleaners are trained and supplied with specialty cleaning supplies that are designed to disinfect all surfaces. Each home is thoroughly cleaned and made ready for all guest before their arrival.

Although we are still stocking our homes with basics including paper towels and toilet paper, supplies are limited. During your stay in Utah. Please plan on bringing your own groceries, and extra paper towels, toilet paper, etc. if you think you will need it.  We have several grocery stores in the area, but bringing your groceries, extra paper towels, toilet paper, etc. will insure you have everything you need for a great getaway and will save you time and frustration in the grocery store, especially during high season.

  • Excessive cleaning after check-out will result in extra cleaning fee based on the severity of the needed clean


GENERAL TERMS AND CONDITIONS

AGE REQUIREMENT: Primary Guest who books and receives keys must be at least 25 years old. A valid ID is required for check-in. Please contact us directly with any questions on this policy.

We will not rent directly or indirectly to guests under 25 years or younger

Guests are required to complete the Happy Guest check-in and provide a photographed copy of their government-issued photo ID, with the Guest’s face, address, date of birth and photo clearly visible and in focus. The reservation billing information must match the information on the government-issued ID of the Guest making the reservation. We reserve the right to refuse service to individuals who do not furnish this information. In the event that a Guest refuses to provide their valid, government-issued ID after making a reservation, the Guest’s reservation will not be granted information to enter the Condo, Townhome or Home and your cancellation of the reservation will be in accordance with the cancellation information on your reservation.

BACKGROUND SCREENING: We may obtain background reports from consumer reporting agencies or otherwise review publicly available information about you, including sexual offender registration lists or databases compiling information about criminal convictions. By making a reservation, you authorize the use of your personal information (including name, address, date of birth, or other identifying information) to obtain these reports, and agree that we may, in our sole discretion, suspend, cancel, block, restrict or terminate your access to our services or reservations made or contemplated, based on our evaluation of such reports.

RIGHT TO REFUSE SERVICE: We may refuse service to any individual for any reason(s) that does not violate federal or state laws. Reasons for refusing service to, or removing, a Guest or any other individual include, but are not limited to: refusal or failure to pay for accommodations; being under the influence of alcohol, drugs, or any other intoxicating substance such that the individual is disturbing the peace; seeking to use the apartment for an unlawful purpose; seeking to bring into the apartment an unlawfully-possessed firearm or any item, including an explosive or hazardous or toxic substance, that is unlawful to possess and that may be dangerous to other persons; destroying, damaging, or defacing the Property or threatening harm to the Property, other guests, or residents of the Property; causing or permitting persons to exceed the maximum allowable occupancy of the apartment; or refusing to abide by the reasonable standards or policies established by the Company, including, without limitation, the House Rules and any published rules and regulations.

RIGHT OF ENTRY: Guests acknowledge, understand, and agree that the Property Owner, Property Manager, Hosts, Maintenance, and other staff reserve the right to enter property at any time to investigate disturbances, check occupancy, check damages, make repairs, alterations, and improvements, as deemed necessary.

ACCESS: You acknowledge that your reservation does not confer exclusive possession or exclusive access. We retain the right to access and enter at any time as needed, however, we will attempt to notify you before entering the Condo, Townhome or Home as a courtesy to you except in the event of an emergency. If called upon, law enforcement will be granted immediate access.

KEYS / ACCESS CODES: Access codes and/or keys are issued to the registered Guest and work at the Check-in time. Failure to return these items at the time of your departure will result in a fee of $500 for each unreturned item.

LOCK OUT: A lockout of Guests from the property will result in a minimum of $25 service call during business hours. Guests agree that a lock out after business hours will result in a locksmith being called and Guests being charged accordingly


PAYMENT AND DEPOSITS: Payments can be made by: Visa, MasterCard, Discover or American Express. All reservations require a 50% deposit/payment to be made at the time of booking and remaining payment to be paid 14 days prior to scheduled arrival date. If booked within 14 days of arrival, the full amount is due.

RESERVATION PRICES DO NOT INCLUDE: Remember that expenses and items not mentioned including but not limited to: transportation, tips for personal services, meals, insurance, telephone calls, and other items of a personal nature are not included.


CANCELLATION POLICY: Please note we will be as flexible as possible with regards to cancellations and date changes. Please reach out with any questions.

Reservations cancelled 30 days or more prior to arrival date are refundable with a $75.00 cancellation fee.

Reservations cancelled 14 – 29 days prior to arrival date are 50% refundable.

**The purchase of Rental Guardian Travel Insurance is advised by Alpine Luxe Properties (See ‘TRAVEL PROTECTION’ below).


TRAVEL PROTECTION: Travel Insurance has been made available as an option with your reservation. Travel Insurance reimburses for pre-paid, non-refundable expenses due to certain unforeseeable circumstances that may jeopardize your vacation investment and force you to incur unplanned expenses.  We strongly recommend you purchase this valuable protection.  If not purchased, you acknowledge you have read and understand our cancellation policy and choose not to purchase Travel Insurance on behalf of all occupants.

DAMAGE INSURANCE:

  • Your damage insurance coverage is up to $25,000
  • Guest is responsible for any damages above and beyond the provided insurance coverage limit of $25,000. ** If damages exceed your damage coverage, you are responsible for all additional charges, costs to make the unit whole and rentable again
  • Lost keys are not covered by your insurance coverage and so a $275 fee will be charge, more if an emergency (determined by the locksmith at the time) will be charged to guest for replacement.
  • If lockbox access is provided with a key inside, please place the key back in the lockbox or it will be deemed lost and you will be charged for replacement costs (see prior lost keys for associated costs)
  • If guests refuse the Damage Waiver, Guests must provide a refundable security deposit, or a non-refundable damage waiver is required.
  • Guests will leave the property in a clean, undamaged condition.
  • Guests will follow check-in and check-out procedures.
  • Guests will follow the pet policy.
  • Properties have been cleaned and inspected prior to Guest’s arrival, with photographic documentation.
  • Guests agree that by accepting the booking and entering the Premises that they are accepting this Rental Agreement Contract.
  • Guests have authorized Happy Guest to charge Guests’ credit card for any repairs, damages, replacement, or any additional cleaning required or purchase Damage Waiver Insurance. Guests’ will be charged the excess damage costs in the event that the insurance doesn’t cover the incurred damage.
  • Guests must select either a refundable security deposit or a non-refundable damage waiver. The refundable security deposit will be held for 7 days post check-out and will be refunded provided there are no damages. The damage waiver is recommended.


RESPONSIBILITY: Tenant hereby assumes all risk of damage to any and all personal property of Landlord and Tenant, including household furniture and goods during such use and occupancy. Tenant hereby assumes any and all risks associated with the foregoing and agrees to hold Landlord harmless from and indemnify Landlord against the same. PERSON(S) RESPONSIBLE FOR BOOKING STAY AND ACCEPTING PROPERTY KEYS MUST BE AT LEAST 25 YEARS OF AGE AND WILL BE REQUIRED TO SHOW A VALID ID AT TIME OF CHECK-IN.

Guests are responsible for any harm or theft occurring at the Condo, Townhome or Home during their stay, whether caused by themselves, their visitors, or other individuals present. This encompasses damage to rooms, fixtures, furnishings, and amenities like towels, electronics, and kitchenware. The Company retains the right to keep the Guest’s payment details and charge the full amount necessary, at its discretion, to cover expenses incurred due to damage or theft. These charges may involve repair or replacement costs, smoking fines, and cleaning fees, among others. Efforts will be made to minimize repair expenses. Should the Condo, Townhome, Home or Property become unrentable due to the Guest’s actions, the Company reserves the right to charge the Guest the nightly rate advertised on the Booking Channel until it’s returned to rentable condition, along with any damages caused. Guests agree to not move or rearrange any furniture, and if they do that, they are subject to being charged an additional fee to put the place back as it was when you arrived. Should the Company find that you have introduced any pest infestation into the Property, it retains the right to levy charges for all associated costs and expenses. These may include urgent response needs, and the loss of revenue, as deemed necessary by the Company to resolve the infestation not covered by your damage waiver insurance.

In the event of equipment breakdown, guests are required to promptly inform hosts for necessary repairs. Please note that there is no assurance regarding utilities like heat, air conditioning, electricity, water, appliances, entertainment systems, gas utilities, etc. No reimbursements or compensations will be provided for any failures in these amenities. If a repair technician is called due to a unit malfunction caused by guest oversight, neglect, or misuse, guests agree to cover the repair costs.

Guests must sign a Rental Agreement and provide ID accepting the following City Codes:
1. Tenant/guest is prohibited from subletting the property rental. Only property owner or agents with valid property rental permit and TOT certificate are allowed to advertise or rent a property.

2. Number of guests must not ever exceed the allowed amount for the property.
3. Number of cars must not ever exceed the allowed amount for the property.
4. No smoking or vaping of any legal or illegal drugs of any kind, are not allowed inside or outside of the property. No tampering with smoke detectors. $1,000 fine
5. No downloading/streaming (ie:BitTorrent) of any copy write material.
6. Neighborhood quiet hours are from 10 PM to 8 AM. During this time No loud noises, No amplified music, No media outside allowed on property at any time. Guests agree that they will not disturb any guests or surrounding neighbors. Guests agree to refrain from making loud noises and excessive television and/or radio volume that may disturb others including Hot Tub use, if any. Parties, events, and other gatherings are prohibited. We actively monitor the premises to ensure compliance with our noise policy. Decibel sensors are in place to alert us of any sounds surpassing 75 decibels (dB) or other location-dependent thresholds. By staying here, you consent to this sound level monitoring. If we receive a noise complaint or detect noise levels exceeding the specified thresholds, we will issue a first warning to reduce the noise. Should there be subsequent noise complaints or breaches of the noise thresholds occur after the initial warning, we reserve the right to charge you a fee and/or remove you from the premises without refund, at our discretion, guest will be asked to vacate property with no refund.

7. No parties or events outside or inside of property at any time.
8. No spa/hot tube use (IF AVAILABLE) between 10 PM AND 8 AM

9. No incidental camping, no over night camping, sleeping in tent or on decks attached to the property unit, no sleeping in travel trailers or recreational vehicles parked on the property.

10. Guest shall not bring personal grills and barbeques to the property.

11. On Red Flag Days (National Weather Service Red Flag Warning) for geographical area where the property rental is located, all sources of outdoor flame, including gas/propane and pellet barbeques are prohibited.

12. No person(s) or guest shall use, discharge, or possess any fireworks on the property, as defined in Section 12505 or 12529 of the State Health and Safety Code.

13. Please notify management if you have lost property key. Lost keys have a $275 replacement fee, maybe more depending on the lock system.

USE: There are no safety personnel available at any time for anything. Guest use of any amenities is at their own risk. No exceptions. The Guest understands that sometimes amenities or services can be closed or out of service, or otherwise cannot be used for any reason, and that no refunds will be made. It is agreed that the premises shall be used only for residential purposes and for no other purpose whatsoever. Tenant in his possession, use and occupancy of the Premises agrees to observe and comply with all restrictions, laws and ordinances affecting the premises or occupancy thereof. Tenant further agrees that no use shall be made of the premises or acts done which will increase the existing rate of insurance upon the Premises or will cause a cancellation of any insurance policy covering the Premises.

NON-SMOKING POLICY:  Please Note: SMOKING OR VAPING IS NOT PERMITTED IN ANY RESIDENCE. A $500.00 fee may be charged for ionization, if smoking or vaping in the property is detected. ABSOLUTELY NO EXCEPTIONS! Smoking is strictly prohibited throughout the entire property and unit, including all decks, porches, and steps. Guests agree to pay a fine for odor removal if smoking occurs in the Condo, Townhome or Home and in the hallways, common areas, or if cigarette butts are found on the premises. Please be aware that any smoking in these areas will result in additional fees for extensive cleaning and odor removal.

Smoking of any kind is prohibited, encompassing tobacco, marijuana, e-cigarettes, vape pens, and hookahs. This prohibition extends to all areas of the Property. Marijuana and tobacco sensors are installed in the Condo, Townhome and or Home to assist in enforcing this regulation.

ALCOHOL/DRUGS POLICY:  No consumption of alcoholic beverages is permitted by individuals under the legal age of twenty-one (21). Guests acknowledge that if they or anyone under the legal age is arrested for underage drinking on rental properties or observed consuming alcoholic beverages, their stay may be terminated, and Guests evicted without any refund or rebate. Illegal drug use is strictly prohibited for all individuals.

OTHER BEHAVIOR: Occupancy and activities on the premises must not disturb or inconvenience neighbors in any way. This includes, but is not limited to, hosting parties, driving at excessive speeds in the neighborhood, creating excessive noise, engaging in obnoxious behavior, and using firearms, BB/Pellet Guns, Paint Ball Guns, or fireworks. The Company reserves the right to terminate the reservation and require Guests to leave the premises immediately, forfeiting all reservation fees. No refunds will be provided.

Other Noise: Guests acknowledge, understand, and agree that nearby attractions and construction can occur. No refunds will be offered for inconveniences due to but not limited to nearby construction noise, town or road repair, and utility maintenance.


OCCUPANCY and PARKING:

No events, including weddings, are permitted on the premises. The occupancy limits are determined by fire code regulations, fire safety guidelines, and local statutes or ordinances. Guests are obligated to adhere to these maximum occupancy limits. By state law, the number of guests may not exceed what is posted for each home. Maximum occupancy includes children over 12 years old and infants. All guests must be listed on the reservation, including children aged one year and older, who are considered occupants. Rollaway beds, inflatable mattresses, and sleeping bags are not permitted. Should you exceed the maximum occupancy stated on your reservation, you will incur a fee and possible termination of reservation without refund.

VISITORS: Visitors must always be accompanied by the Guest. The Guest is responsible and liable for the actions of all Visitors.

CHILDREN: Children of all ages are welcome. Guests are responsible for the actions of all minors who accompany them on the Premises.

NO MAIL AND NO PACKAGES: Packages will not be accepted, and no mail deliveries can be sent to the address. Guests agree to have their mail and packages sent to a nearby UPS store, FedEx store or another provider.

NO FIREARMS AND WEAPONS: Guests are not permitted to carry firearms or weapons on the Property.

EMERGENCIES: In the case of an emergency, call 911 immediately


Parking is restricted to the number of vehicles legally allowed at each property and vehicles must be parked in driveway or garage (if available). No street parking is allowed. You will be towed or fined. Vehicles must be parked as described in the instructions provided. Vehicles are parked at the risk of the vehicle’s owner. We do not assume liability or responsibility for the vehicle, the occupants, or any contents while the vehicle is being operated or while it is parked on the property. If the vehicle is left at the property past check-out time, the vehicle is at risk of being towed. Winter parking restrictions will apply, Snowplows and other snow clearing vehicles may cause significant damage to your vehicle, please see rules and regulations on this as we are not responsible for vehicles damaged, stuck or towed at any time but specifically during winter conditions

PET POLICY:

Unless this is a Pet friendly home, NO pets are allowed.
Pet friendly properties are allowed to have up to two dogs on the premises, unless otherwise specified. There is a non-refundable fee of $50 per pet that will be added to your reservation. If you are bringing pets, be sure to select ‘yes’ and ‘1’ or ‘2’ in the Pet Fee section of the Reservation Options when completing your booking. Please contact us with any questions regarding your pets. Properties which are NOT pet friendly have been deemed so at the property owner’s discretion due to damage or allergy concerns. Guests who bring pets to a property that is NOT pet friendly will be charged a $500 fee to cover carpet and upholstery cleaning. 


Please note that we do have Deer, Cayotes, Bears, Mountain Lions, Bobcats, Moose and other wildlife in this area. Watch your pets carefully! Pets are your responsibility and should be in a crate while left alone at the property. Pets should always be secured within the boundaries of the property and if property is not fenced, pets must be kept on a leash and always accompanied by the owner while outside. You are responsible for cleaning up after your pet – inside and out. Thank you!

SERVICE ANIMALS: Service Dogs are welcome with the following Service Dog Policy: Service Dogs with a history of violent or aggressive behavior towards individuals or other pets will not be accommodated. Your service Dog must comply with all licensing and vaccination requirements mandated by the state and local jurisdiction where you permanently reside. Upon request, you must provide copies of your service Dogs current licensing and vaccination records. Please note that “licensed” and “licensing” here refers solely to the basic registration of Dog ownership, not to any certification of your animal as a service animal. Your service Dog must never be left unattended. Guests will make advanced notice that they will be bringing a service Dog as part of your reservation and confirm your compliance with all service Dog-related regulations specified on the Booking Channel. Service Animals in Utah do not include any animal other than a dog, trained or untrained; or an animal solely to provide; a crime deterrent; emotional support; well-being; comfort; or companionship (UCA 62A-5b-102).

INTERNET/WIFI POLICY: While we make best efforts to provide interruption-free, high-quality Internet access, we assume no liability in the event that Internet access becomes unavailable. If Internet/WiFi is provided at the property, guest must not, UNDER ANY CIRCUMSTANCES use the provided internet to download or stream any copywritten material (ie:BitTorrent, etc.). Our ISP does track illegal activity and guest will be charged $500 for each copyright infringement instance logged during their stay. Guests are prohibited from using WiFi for any illegal activity, including, but not limited to, violating the Digital Millennium Copyright Act. The Company assumes no liability for Guest use of the Internet.

CHECK-IN AND CHECK-OUT TIMES: The check-in time is after check-in time on the first day of your reservation. Sometimes early check-in is available and is easy to purchase on your Happy Guest reservation page. Normal check-in time is 4 PM and check-out is 11 AM. Check-out for larger homes may be 10 AM if deemed necessary by Alpine Luxe Properties. The check-out time is before check-out time on the last day of your reservation. Sometimes late check-out is available and is easy to purchase on your Happy Guest reservation page. Late fees will be charged for departures after check-out time that did not purchase a late check-out.

EARLY CHECK-IN / LATE CHECK-OUT: Early check-ins and late check-outs are sometimes able to be accommodated with an easy purchase on your Happy Guest reservation page. If you have a need to check-in early or check-out late, you can also consider adjusting your reservation dates to be one day earlier or later, so you have the place for as long as you need it.

ARRIVAL/PROPERTY CHECK: Walkthrough: You, the tenant, are responsible for any unreported damage to the property. Please walk through the home when you arrive. If you notice any noticeable damage or if something is not working, please notify our Property Manager immediately so that we can correct any issues (if possible) and so that you are not held responsible for it. If anything does get damaged during your stay, please report it immediately.

Wildlife: Remember that Utah is a mountain/forest community and animals, rodents, and insects are a part of that. Your rental property is inspected before your arrival; however, we cannot guarantee that the property will be free of pests. Cobwebs can materialize in a few hours and are a constant issue in the woods, and insects (dead or alive) can show up quickly in an empty home. We cannot offer credits for these situations but contact us with any issues and we will send someone over right away.

Snow Removal: (If in a Single Family Home) While we do our best to have the driveway cleared of snow (in winter) for your check-in, delays can happen if snow occurs just prior to/during your arrival or if city plows leave a berm (snowbank) blocking the driveway. We clear snow from driveways and paths to entry after 6 inches of snow accumulates. Each home is supplied with at least 1 shovel. You will be responsible for keeping areas cleared of snow during your stay. Please contact us if assistance is required. No credits can be issued due to inconvenience with snow removal.

SUPPLIES: Each of our rentals are supplied with clean linens on the beds, pillows, bath towels for the number of guests, toilet paper, paper towels, trash bags, dish (and dishwasher soap when necessary) soap, sponge, and hotel-style shampoos, soaps, and lotions. Kitchens will have a coffee maker, toaster or toaster oven, dishes, utensils, glassware, cookware, and other kitchen basics. Other supplies will vary by property. If your home has a spa, you may want to bring beach towels for your guests. Firewood is available for use at a few homes (see home description/amenities list), but generally you will need to provide your own. Please contact us with any questions.

FEATURES: We do our absolute best to make sure that all home amenities (spa, game table, BBQs, TVs, internet, cable, etc) are ready and working upon your arrival. If something stops working during your stay, notify us and we will do everything possible to get it working right away. No amenity is guaranteed, and we cannot issue refunds or cancel due to an unusable amenity. Please note that it may take some time for the refrigerator to cool after you add items because it was empty prior to your arrival. If your property has a propane BBQ and you run out of propane, please contact us and we will bring over a full tank.


CHECK-OUT/DEPARTURE:
Please do the following before you leave. Check-out time is by 11 AM, unless you have made prior arrangements with us for late departure.


1. Wash all dishes and put them away or run the dish washer, straighten house and check for forgotten items.
2. Turn the THERMOSTAT to 58 degrees, if applicable. Please do not turn the heat any lower than this.
3. Clean up after your pets – inside and out. (Pet friendly properties)
4. TAKE ALL TRASH (Including cans and cigarette butts from outside) to the designated trash/bear bin located outside of the property. *We will charge you $25 per bag of trash left in the property.
5. Turn off all lights, close and lock all windows and doors.
6. Call the property manager before leaving at (530) 302-3555 or Email: [email protected]

You may be charged an additional hourly surcharge of $60 if the above procedures are not followed and additional time is required by the staff to make the home ready for the next guest.


ITEMS LEFT BEHIND:
If you find that you have left an item behind, we will gladly ship it back to you. Contact us as soon as possible, so that we can find the item before the next guest arrives. We will charge you a $25 handling fee plus the cost of shipping and any desired insurance.

GENERAL:

LICENSEE OF OWNER Guests are considered licensees of the Owner, not tenants, and therefore do not acquire any interest in the property. This means Guests do not hold a lease or sub-lease of the property but have a license to utilize the Premises as long as they adhere to the terms of this agreement.

NO TENANCY This agreement does not constitute a lease or residential tenancy agreement and does not grant any property, legal, or beneficial rights or interests in real estate to you. You are not a tenant and do not possess exclusive possession of the Property. Guest agrees this is not intended to be their permanent residence.

USE OF PREMISES Guest agrees to not use the property for any unlawful or illegal purposes. Guests are prohibited from driving nails, screws, or other items into any portion of the apartment. The property is designated for vacation purposes only, and Guests must refrain from conducting any commercial, industrial, or retail activities on the premises.

REMOVAL FROM THE PREMISES Guests acknowledge that the Company or other lawful authority may remove Guests or any visitors without notice for various reasons, including failure to vacate the property at the designated departure date and time, non-payment of reservation fees, violation of any rules in the House Rules, violation of any rules of the housing development, condominium association, or engagement in any unlawful acts. Guests agree to hold the Company harmless for any incidents necessitating the removal of Guests or visitors from the Property.

LISTING INFORMATION:  Information provided regarding individual listings is believed to be accurate but cannot be guaranteed.

CHANGES OR MODIFICATIONS TO HOUSE RULES

The Company reserves the right to modify or amend any of the House Rules in writing at any time without prior notice. Any written modification or amendment of the House Rules prior to your departure is deemed incorporated into the Agreement governing the reservation. Guests should regularly check the reservation details for updates to the House Rules.

MISCELLANEOUS

ARBITRATION: Any dispute arising out of or related to this Agreement will be submitted for and resolved by binding arbitration before the American Arbitration Association (AAA). The Company may seek temporary restraining orders, temporary or preliminary injunctive relief, and/or declaratory relief from a court of competent jurisdiction. By entering into this Agreement, you and the Company waive the right to a trial by jury or to participate in a class action.

CONFIDENTIAL: The entire arbitration proceedings shall remain confidential and not be disclosed to anyone other than the parties to this Agreement, except as required by law.

LIMITATIONS PERIOD: Any claims arising out of or relating to this Agreement must be commenced within one year from the occurrence of the facts giving rise to such claim or action, or such claim or action shall be barred.

CLASS ACTION WAIVER

You agree not to file a class action or participate in a class arbitration against the Company.

GOVERNING LAW AND VENUE: This Agreement shall be governed by the laws of the State of Utah, United States. Any dispute not covered by the Arbitration provision may be filed only in the state or federal courts located in the State of Utah. You waive the right to a trial by jury.

DISCLAIMER OF WARRANTIES: The Company disclaims all warranties to the fullest extent provided by law.

LIMITATION OF LIABILITY: The Company’s liability is limited to only the amount Company received. You agree that this is less than what you paid because there are many other service providers included in any transaction and the Company receives only a certain amount after those other providers take all their fees.

WAIVER AND SEVERABILITY: Provisions of this Agreement are intended to be enforced to the fullest extent permissible under applicable law. If any provision is held invalid or unenforceable, it shall be modified to the greatest extent permissible, and the remaining provisions shall remain in full force and effect.

MISCELLANEOUS: The Company reserves the right to move Guests to a comparable property under certain circumstances beyond its control.

By signing this Agreement and/or making the required payment, you acknowledge and consent to all terms and conditions of the House Rules, on your behalf and on behalf of all members of your party. By signing below, I agree to all terms and conditions of this agreement. I hereby give permission to charge my credit card for the amounts and dates listed.
